All medical, dental, and vision participants should have their claims filed according to the instructions as indicated on the back of the participant’s insurance ID card.
This is generally completed by the provider. Participants for our Self-Funded medical/dental groups will be directed to contact our office for claims filing procedures.
Section 125/HRA Claims should be faxed, mailed, or emailed to the contact infomation indicated on the bottom of the claim form.
